By accessing and using Forms.fyi, you accept and agree to be bound by the terms and provision of this agreement. If you do not agree to abide by the above, please do not use this service.
We provide unique form endpoints that accept HTTP POST requests from your websites and applications. Each endpoint can be configured with validation rules, notification settings, and custom responses.
Form submissions are stored securely in our database with metadata including timestamps, origin verification, and IP hashes for spam protection.
Optional email notifications can be configured to alert you when new submissions are received. Webhooks are also supported for integration with other services.
If you enable file uploads for a form, visitors may attach documents and media within the limits and types we allow. We apply technical restrictions (such as file type and size limits) to reduce risk, but we do not guarantee that uploaded files are free of malware, viruses, or other harmful content. Files are stored so you can retrieve them; you are responsible for how you download, open, and distribute them.
You must provide accurate information when creating your account and forms.
You are responsible for ensuring your forms comply with applicable laws, including privacy regulations like GDPR and CCPA.
Keep your account credentials secure and immediately notify us of any unauthorized access.
You may not use our service for illegal activities, spam, phishing, or any malicious purposes.
You are solely responsible for the content submitted through your forms, including any files uploaded by your visitors. You must comply with applicable laws (including privacy and data-protection rules), obtain any required consents, and only collect data you are permitted to process. If you enable attachments, treat downloads with the same care you would for files from an unknown source.

Free accounts are subject to monthly submission limits. Paid plans offer higher limits and additional features. We strive to maintain 99.9% uptime but do not guarantee uninterrupted service.
We reserve the right to suspend or terminate accounts that violate these terms or abuse our service.
Forms.fyi is provided "as is" without warranties of any kind. We shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including without limitation, loss of profits, data, use, goodwill, or other intangible losses.
Without limiting the foregoing, we do not warrant that uploaded files are free of viruses, malware, or defects, and we are not liable for loss or damage arising from your opening, storing, or forwarding of attachments submitted through your forms.
We reserve the right to modify these terms at any time. Changes will be effective immediately upon posting. Your continued use of the service constitutes acceptance of any modified terms.
